Last year we formed our very own Patient Reference Group (PRG).
The group consists only of our registered patients and is representative of our registered patients. We advertised for members to the group extensively, wishing to attract as many different age groups, ethnicities and backgrounds in order to be truly representative of our population.
Once established, we agreed with our PRG the areas to be covered in our current local practice survey.
We are now asking patients to take part in our annual survey which will run in practice until Friday the 2nd March 2012. You can download a copy of our practice survey below. You can complete it and either email it back to sueshone@nhs.net or hand it in at reception.
We will publish our findings on our website, in the practice and in our newsletter.
We will then provide our Patient Reference Group with the opportunity to discuss our survey findings and we will together reach agreement on any changes to our services that may need to be made. After this an action plan will be agreed with our PRG and we will work together to implementing any changes.
All actions taken will be widely publicised along with our subsequent achievement.
